Black History Month: Celebration on the Mountain

I, the undersigned participant or, if the participant is under 18 years of age, the participant’s parent, legal guardian, or authorized individual, give permission for the participant named above to participate in the Black History Month: Celebration on the Mountain event organized by BSL Educational Foundation Inc., including snow tubing and related activities at Powder Ridge Mountain Park & Resort.

I understand that snow tubing is a physical activity that involves inherent risks, including but not limited to slips, falls, collisions, weather-related conditions, or equipment use. I acknowledge that trained staff and venue personnel will be present to provide supervision and that all participants must follow posted rules, safety guidelines, and instructions.

I grant permission for BSL Educational Foundation Inc. to use photographs and/or video of the participant taken during this event for promotional, educational, or informational purposes, including use on websites, social media, and printed materials.
